Sew 2 of the patches together, stopping 1/4" away from the edge of the fabric, and backstitching a few stitches. Push the seam allowances to one side, and sew the 3rd patch to one side of the Y, stopping your seam right before the needle pierces the stitches from the first seam. (The edge of the 3rd patch will extend 1/4" past the intersection of the 2 seams.) Sew the other side of the Y the same way, making sure you don't sew into or past the other seam lines.
This depends on the pocket. You may be able to iron the raw edges of the material under (towards the "wrong" side) and then sew along the side and bottom edges to secure the pocket. If you want a pocket hidden along a seam you will have to assemble the pocket, and attach it to the item as you sew the seam or waistband it is attached to.
Print out the template, trace on the shapes of template, and cut the shapes out from the fabric. Then draw two lines with fabric paint or glue on two thin strips of black felt on 1 piece. Then pair the pieces up, good side to good side. Sew up 1 side. Then open up the two pieces and pair them up, good side to good side. Sew all the way around after that. Then take the circle that you traced and sew it on, leaving a small gap. Use the gap to flip it inside out and stuff it. Sew it up with a ladder stitch.
