If you are speaking of cartilage, then I would advise that you go slowly and don't overdo it because it has a very common tendency to "bunch up" as it were on the back of the ear and looks like a bit of a keloid, which isn't very attractive. If you were to want a large gauge in cartilage than the best route to go is to have a mod artist use a dermal punch, but do keep in mind that the cartilage DOESN'T regrow, so the hole that you get will be the hole that you are stuck with.
