It would be at the discretion of the shipper. Flat rate could be either USPS, UPS, Fed-Ex or whatever method of shipping is chosen by the shipper for a "Flat Rate". Also, USPS has flat rate boxes and envelopes.
Shipping boxes typically come in 3-5 different sizes. Each size has a different purpose, delivery time, and price. These often fit the small, medium, large, extra large, type of size chart.
Not sure what a chipping case is. Shipping cases come in whatever size is needed. There is not standard size. Some of the more popular sizes are 18" x 12" x 16", 24" x 14" x 18" boxes.
