All they need is temperatures above the freezing point of water, and about a teaspoon of still water to lay eggs in for the larvae to hatch, they come out hungry and seem to be able to fly an obscenely long way before they have to feed on blood. Then again, they will feed on anything that doesn't eat them, and one study I have read seems to indicate that they can even drink plant sap once in a while. I'll see if I can find it again and include it as a link.
Mosquitoes lay their eggs in stagnant water. They normally infest ponds, swamps, and marshes. Mosquitos can live in a variety of locations, but mostly prefer hot, humid environments.

Mosquitoes hatch around any form of standing water, and the weather helps them hatch. They have no preferences in who they bite, as they need blood to survive.
