I would say:
1) Mechanical. The park relied too much on automation. Computers malfunction and break down, and so cannot be heavily relied upon to keep the park under control. When John Hammond remarks that all major theme parks, including Disneyland, have delays, Ian Malcolm replies, "Yes, but John, when the Pirates of the Caribbean breaks down, the pirates don't eat the tourists."
2) Human. They relied too much on Dennis Nedry, who turned out to be a crook. As Sam Jackson's character says, "John, I can't get Jurassic Park back on line without Dennis Nedry." If they had hired and trained more people who could take care of the computers for them, instead of relying solely on Nedry, they might not have had this problem.
3) Mathematical (chaos theory). As Malcolm says, absolute control is never possible, because there are too many unforeseeable variables. It's unrealistic to expect to be able to keep a population of man-eating dinosaurs in check. Perhaps if they had only bred herbivorous dinosaurs, they wouldn't have had this problem, but Hammond was intent upon breeding the more exciting -- meaning, more dangerous -- species, like T-rex and velociraptor.
