Which states are completely 45 degrees north?
Montana, North Dakota, Washington, and Alaska are entirely north of 45° north latitude.
Whether they're above or below that parallel depends a lot on how you hold your map.
If you bought your map in Indonesia or Australia, then it's printed with south at the top, and 49 of the 50 United States are completely above 60 degrees north latitude, and part of the the 50th one is too. If you bought your map in North America or Europe, then it's printed with north at the top. In that case, no states are completely above 60 degrees north, and only part of Alaska is.
Louisiana is above the equator. No part of the continental US is below the equator. This assumes that you are comfortable with the completely arbitrary convention of calling north above and south below.

That's going to depend on which way your map is drawn. If your map is printed in central Australia, with south upward, then 49 of the states in the USA are entirely above the 60 N parallel, and part of the 50th is too. If your map has north upward, then only one state ... Alaska ... has part of its territory, but not all, above the 60th parallel north, and no other state has any at all.

If you're referring to the 50 states of the USA, then every one of them is totally and completely north of 6 degrees north latitude.

If you mean how many of the United States of America, then the answer is all of them are north of the equator.All 50 states in the United States are north of the equator. The state of Hawaii is closest but is still several degrees north.
