It depends which ending you chose, but I did make some notes of the route I took, and perhaps they may be of help to you:

I am not sure if you wanted to know how the whole mission works, but here goes:

If the route you have chosen takes you to Out Of Commission as the final mission, initially the car chase from Alderney City to Pegorino's hiding place is just a case of keeping on the tail of the mobsters' car, keep them in sight and Dodge traffic as best you can.

Once Niko's car arrives, you have to take out a host of mob heavies, outside and inside, to get through to Pegorino hiding in the old Casino. There is no timer on this, so you really can take your time getting rid of them in whichever way seems to be best. It isn't difficult, because there are plenty of places overlooking from the road for example, but keep Niko behind something whenever he is firing, because of the returning firepower. Try to pick up their weapons when you can. Once Niko has managed to get past the handful of heaves inside the first part of the Casino (be careful particularly of the one just inside the door to the left - try and keep Niko firing from behind the door as much as you can until he goes in), and has connected with Pegorino, the mob boss will do a runner with more heavies firing at Niko, again just keep Niko ducked down or behind something, as he fires his way through. Pegorino will take to a speedboat. When Niko has fought through the last group of mob bodyguards, including a car that will pull up just before the jump down to the jetty (best to just run from them) he will find a Sanchez dirt bike, waiting on the end of the jetty. Niko has to hug the coastline and chase the speedboat. It is difficult, boggy terrain, but eventually he will see a ramp which he has to go up at speed in order to jump up on the helicopter which is hovering overhead, containing Jacob and Roman.

Once he has managed to get on board and started piloting the chopper, Niko has to avoid being hit by an RPG from Pegorino's speedboat too early. The chopper will eventually get hit anyway, but then the action goes into a cut scene over Happiness Island as Pegorino's boat and Niko's helicopter land there. Niko then has to chase after Pegorino into Happiness Island. The one thing I have avoided doing is stopping to fire at anyone in the way, whether it be bodyguard or cop, as long as Niko has good health he should be able zig zag through any bullets until he gets to Pegorino in the interior, although Niko will take some hits of course. Probably best for Niko to carry an SMG so that he can run faster and also be able to take out Pegorino. If Niko is successful, the action should go into a cut scene with Pegorino's dying moments.

Useful tip: Before Niko connects with Pegorino in the casino (as there is no timer on this mission), take Niko back up the road to the garage on the hill, behind the Casino complex. There is a building next door to the left with a ledge at the back, and a rubbish skip on wheels which Niko can get up on to get at the armour hidden on the ledge above. This will top up his armour before the worst of the action. There are two first aid boxes inside the Casino itself, so that will help too.
