[despite the fact that this is under "rhetorical questions" I will answer it anyways] Man-holes are round because no matter which way you turn the lid, it will never fit into the round hole (With a slightly smaller diameter). If it were any other shape, like a square or triangle, it would be possible to drop the lid in the hole by turning it diagonally.

depends on the size of drain piping. Most toilet either have a 3" or 4" drain. The size hole needed would be larger than the outside diameter of the piping. 3" pipe=4" hole / 4" pipe=5" hole
It is easy if the diameter is less than the width of the square; impossible if larger.
The question cannot be answered in a sensible way. The size of the hole is given in only two dimensions - in all likelihood, that means the area that the hole covers but not how deep it is. The amount of dirt required to fill the hole will depend on the depth of the hole as well.
