The sinking of the Maine, in Havana Harbor, was thought (by many scholars now) to be due to scuttling of the ship (rather than an attack by the Spanish), in order that the US could "legally" declare war on Spain.

It remains quite unlikely that the Spanish sank the Maine on 15 February. It had been in the Harbor for three weeks when the explosion happened, and nine weeks after the sinking Spain declared War. The sinking of a single ship without other hostile activity seems does not appear to be an act of war. It was not a false flag action and it is also unlikely that Cuban Rebels could have been involved.

The sinking of a hospital ship
It was an enemy ship and it was carrying munitions. When it was hit by a torpedo, some of those munitions are thought to have exploded causing more damage and hastening the sinking of the ship.
The one major warning sign at Pearl Harbor happened at approximately 6:30 that morning. As a ship entered the harbor a sailor on another ship thought he say a periscope trailing behind the ship. They were able to contact a nearby ship - the Ward - who confirmed seeing signs or a sub as well. They fired on the submarine sinking it. When the message was sent it was sent encoded that they had 'fired upon' an enemy ship. The telegram did not state straight out that there was a successful attack on another ship. If it had, if it had not been encrypted - those in harbor may have had a chance to prepare for the attack.
