A World War 2 pillbox was a concrete shelter for machine guns used by both the Axis and allies. They can still be found all along the south and south east coast of England and were used to shelter and house the guns and several occupants. They were used to defend beaches most famously and often inflicted numerous casualties on attackers, such as at Omaha beach were 2,400 American troops were killed on D-Day. However, pillboxes were also used to defend high value places and open fields where the gunner could ambush enemy troops.

I stored my pills in a pillbox.
Maniac was likely referring to the racism and prejudice in the McNab house, which would have created a toxic environment. The smell represents the hatred and intolerance that permeated the household, making it a place filled with negative energy and hostility.
The McNabs are building the pillbox to keep their family safe from Maniac Magee, who has been befriending their son and causing tension in the neighborhood due to his ability to bridge racial divides. This is a way for the McNabs to isolate themselves and protect their family from potential conflict or influence from Maniac Magee.
