Carlisle was the first, and kept his name, Carlisle Cullen. He then created Edward, whose original name was Edward Anthony Mason. Soon after, Carlisle changed Esme (his wife), who was nearly dead. Next was Rosalie, who kept her name, Rosalie Hale (soon to be King until she almost died and was changed), to disguise Jasper and herself as the adpoted twins. Rosalie found Emmett being mauled by a bear and she had Carlisle change him, but they don't say what Emmett's last name was. Alice and Jasper found each other and went to go find the Cullens based on a vision that Alice had. The way Alice was changed was by a vampire working in a mental hospital who had gotten attached to her. He changed her to keep James from tracking her. Her name was Mary Alice Brandon.
