Czechoslovakia, before the break-up.
Czechoslovakia was splited in 1993 in two independent countries - Czech Republic and Slovakia. However, both countries joined Schengen Area in 2007 and therefore you can travel between them without passport control (random checks may occur).
Czechoslovakia existed between 1918 and 1938, between the wars. Sudetenland, basically all of the Czech part, was granted to Germany in 1938, and the remainder of the Czech part was later occupied entirely. A puppet state of Slovakia formed, splitting the country apart. Czechoslovakia was reformed after WWII in 1945 and lasted until the fall of communism in 1992, led by a peaceful split into the countries of the Czech Republic and Slovakia today.
There are approximately 196 countries in the world today. The last country that was formed was South Sudan on July 9, 2011.

Slovakia became an independent state twice. Firstly in 1939, when (First) Slovak Republic aka Slovak State was formed after Czechoslovakia dissolved, when Adolf Hitler took today's Czech Republic as a Protectorate of Bohemia and Moravia. Czechoslovakia was reinstituted after the war in 1945. Second time, when Czechoslovakia broke into Czech Republic and Slovakia, was on 1st January 1993.
Only one: Israel (the borders were different than today's borders though).
Yes a Few .

The four countries founded the international governing body and still hold membership today. In other sports the separate countries did not seek separate membership.

I'm not clear on your question, but it was called Pangea before it split. The super continent PangeaAbout 180 million year ago there was only one continent called Pangea, that began to break up into separate continents. Over time as the land masses moved apart it formed the continents we know today. They are still moving. There were no countries 180 million years ago. Countries are boundaries, as man divided the land into separate territory.
