A:Until the fifth century and beyond, the majority of Christians lived in the eastern part of the Roman Empire, and this was the region that provided much of the theology and leadership for the Church. When Emperor, Constantine (306-337) gave the Christian Church state patronage, he made it clear that he would be the pontiff and leader of the Church. He approved the appointment of bishops and called the Council of Nicaea, to resolve differences of doctrine and ritual within the Church. Constantine's Christian successors maintained their roles as leaders of Christianity. When Theodosius declared Christianity the official religion of empire in 381, he was able to define this as being Christianity as taught "as taught by the bishops of Rome and Alexandria."

The Council of Nicaea had carved the Roman empire up among 4 super-bishops or Metropolitans: Rome, Alexandria, Antioch and Jerusalem. Provision was made later for Constantinople and Carthage. In principle, no bishop could be appointed without the Metropolitan's permission.

Ever since the time of Anicetus in the second century, the bishop of Rome had claimed to be the successor to Peter and therefore in some way superior to, or at least "more equal" than, his peers. Over the centuries that followed, the eastern leaders ceded the bishop of Rome more status, but never as the unchallenged leader of the Church. This came to a head in 1054 when the Pope attempted to change the Nicaean creed without the authority of the bishops, and resulted in the Great Schism.
