The end of the Persian Empire, for a start, although whether that was a bad thing or not is debatable. And... The warring factions that arose after his death. His empire was torn into pieces and fought over. Much better to have let the Persians keep it, I think.

Since Alexander the great and his army came from Macedonia, the Macedonian culture was spread to many countries in the Mediterranean region. The practice of men shaving their beards was also originated by Alexander the Great.

Alexander the Great's defeat of the Persian empire was novel in Greek history and caused several consequences. Culturally Greek and Eastern customs merged together. By the end of Alexander's life he wanted to continue into modern day Pakistan but his army revolted. They were losing faith in his leadership due to the length of time they had been campaigning and by the absorption of Eastern men into the army.

His early death precluded direct effects. His general fought over his empire and established their own kingdoms out of it. Greek influence was dominant until the spread of Islamic arabs took control800 years later.

The biggest consequence of Alexander the Great's death was the crumbling of his empire. Alexander did not appoint an heir and the empire became divided which made it susceptible to attack.
