Before area codes, long-distance calls were completed manually by operators. You would ring the long-distance operator and tell her (almost always a woman) the city and number you wanted to call. The operator might have to consult the "rate and route" book if you were calling a smaller town, and might also have to string together multiple hops from long-distance center to long-distance center to connect your call. For that reason, the cost was far higher than today, even before you factor in inflation.
