The majority of residential toilets out there have the suction to pull down and dispose of a pair of briefs or a small pair of boxer shorts easily. If a pair is flushed, a follow-up flush or two to assure they move out to the sewer line is advisable to expedite their disposal/demise.
The toilets currently being sold for residential use that are labeled 'no clog' will easily take down and dispose of briefs, boxer briefs or even a grown man's boxer shorts in one flush. They are ideal for teenager's bathrooms as they can take down anything that will fit through the opening at the bottom of the bowl, no questions asked. No clogs, ever.

Depends on if it needed to be flushed or not.
You will probably clog your toilet or pipes if you flush anything but toilet paper. I would not recommend trying to flush underwear down the toilet.

Cotton buds SHOULD NOT be flushed down the toilet. Anything flushed down the toilet that is excrement or toilet tissue my block the sewers and cause backup of sewage effluent into homes.
