Unplug connection at fan and check for power ( with engine hot and key on), no power: check fuse, if good it's the relay or temp sending unit could be the problem. power : fan motor bad, you can double check by running a jumper wire to the fan motor ( both POS & NEG)

The fan sensor is bad or the fan relay is stuck. Swapping out the relay is the most likely fix.
Check the run capacitor, if defective, can cause the fan motor to run hot and stops till cools off, on n off. And the sound is a little different while running, it is hard on the motor.
A faulty belt tensioner and/or a worn belt. A run down battery could add to it.
A thermostat probably.

The cooling fan will run whenever the AC or Defroster is on. Beyond that, it will run whenever the sensor indicates that the temperature is above a set-point.
The fan clutch may be at fault or if the fan shroud is damaged or missing may cause those syptoms.
The fan will ALWAYS run, regardless of the temperature. It may speed up as the load increases, but it should NEVER stop. This will cause the CPU to overheat and fry.
Check the voltage to the fan. Possibly a defective temperature sensor located inline on the fan's wiring. Steve J.
my 1976 corvette is supped up and she will run a solid 165 it will probably do more but i dident want to push it cause the nose was coming off the ground
the cooling fan comes on at 230 and normal engine temperature is usually around 218 depending on the outside temp.
