If you cheated with this ex-coworker and doing this then he is jealous of your relationship and yes it is blackmail/extortion. You need to deal with this ex-coworker as it seems he has some issues and may not just go away - this is sick behaviour that could get out of hand down the road and for your safety I would suggest going to the police . As for your relationship all you can do is tell your fiance the truth, how you feel and take things from there. Be careful.
