The opposite of instruct (teach) would be to learn. The opposite of instruct (order, direct) would be to obey.
I would use "instruct" in a sentence like this: "The teacher will instruct the students on how to conduct the experiment."
Visually, a patient will show "oxygen hunger", the use of the muscles of the upper chest and arms to help expand the chest, a cyanotic tinge around the lips, maybe a gray color to the face. Audibly there may be wheezing or the listener can clearly hear the patient making an effort to breath.

You need to create a hole through the chest wall to relieve the pressure. You can do this as you would insert a chest tube by making an incision through the lateral chest between ribs. If the patient is able to cooperate, you would want the patient to breathe in fully, and then seal the hole while their lung is inflated. If I did not feel confident to carry out rescue breaths on a non - breathing patient I would start chest compressions until the emergency services arrived.

The right middle lobe is heard best in the right axilla while standing behind the patient. The anterior chest would find the right middle lobe between the sternum and the nipple.
