This is kind of opinionated. Early symptoms of Smallpox are Vomiting, Aches, High Fever, and Tiredness, while later symptoms are rashes and white legions. If I had to pick, I would say the white legions on the body are the worst symptom, as they can cover 80 percent of the body.

Edward Jenner developed vaccine against smallpox in 1798. He used cowpox virus containing fluid for the same.
In 1796, he injected an 8 year old boy with cowpox, a less lethal form of the small pox. This created an immunization to smallpox and was coined the first vaccination.
You would die around two weeks after infection, the first week you are rather well, and can't infect anyone, but after that your health goes downhill until death finally meets you. The only cure is Vacination, nothing else, so protect yourself from it so you don't get it.
