3 astronauts did die on January 27, 1967 during a test for the Apollo/Saturn-204 launch, also known as AS-204. Command Pilot Virgil "Gus" Grissom, Senior Pilot Edward H. White, and Pilot Roger B. Chaffee died when fire broke out inside the capsule.

Dr. Ronald McNair was one of the seven astronauts that died on the Shuttle Challenger. It exploded 74 seconds after takeoff in January of 1986.
Well, some astronauts do. Not all astronauts will die in space, but some will if the mission went wrong or if it is just being there when you die of natural causes.
