D&D was first published in 1974, originally designed by Gary Gygax and Dave Arneson.
It originated in 1971. I learned to play in '76 and at that point there were only three small pamphlets available. It was 1978 when the first hard cover books came out.
The original creators are Gary Gygax and Dave Arneson. The first publisher was "Tactical Studies Rules", which became "TSR Hobbies", in the early 1970's. The current publisher is "Wizards of the Coast", a subsidiary of Hasbro. They are responsible for creating the Third and Fourth editions of the game (and are presently working on a fifth edition).

E. Gary Gygax was indeed a Jehovah's witness for several years. His wife encouraged him to convert, but Gary was disfellowshipped in the 1970's because his hair was "too long" (below the collar, as was the current fashion) and he was unable to quite cigarette smoking. He did try to quit cigarettes many times throughout his life, but was unsuccessful.
