You need to check the EGR valve, It may be opening to soon.
Have you check the timing???
sounds like a bad throttle position sensor, if it runs ok when faster than that
Check your air filter and throttle position sensor
Can usally be blamed on crank position mabye, maybe not but usally it is.

Change the plugs and wires. also check the throttle position sensor or have it tested
