Christmas gift tags usually have "TO" and "FROM" written on them. The words help to show where to write who gets the gift and who the gift is from.
To make Christmas gift tags cheap and effectively all you need to do is cut small squares out of wrapping paper and write "to" and "from" on them. Use a little tape or glue to stick the squares on your gift and write the name of who it is "to" and who it is "from" on it.

I would think that if a gift was not originally intended for you, and you are certain it was first given to someone else, you do not need to write a special note. You should personally thank whoever gave it to you, however, since that person meant well and thought you would enjoy the gift more than they did. But if you are sure you were not the original recipient, I do not believe a thank-you note is expected.
