To put it simply, new species can form when organisms within a population become so genetically different they can no longer reproduce with one another without generating harmful mutations. This can be caused by genetic drift and geographic isolation. Two populations of the same species who live in different parts of the world will diverge and go on separate genetic paths, until eventually the two populations become genetically different and branch off into two different species.
If two sub-populations of a species become reproductively isolated, it means that adaptation or genetic drift can make the two populations diverge genetically. If, as a result of this divergence, upon reintroduction, members of the two sub-populations no longer reproduce successfully, the two sub-populations have become different species.
It usually is some sort of physical barrier like an large space between islands.
Once species have truly become distinct, members of the two species are no longer able to successfully breed with each other and produce fully viable offspring. This may be as little as a physical separation that prevents contact (e.g. new mountain ranges, isolation on islands), to changes in pheromones that individuals produce so that individuals of the two species are no longer attracted cross species even if they encounter each other, to as much as the development of complete genetic incompatibility (e.g. different numbers of chromosomes in the two species).

The origin of new species. To be considered a member of a singe species, organisms must be capable of reproducing and producing viable offspring. Once they have evolved to become reproductively isolated, they can no longer fulfill these requirements and are considered separate species.
If there is no gene flow between two populations then they will eventually become genetically distinct, thus forming two separate subspecies. If this isolation continues for longer still, then the 2 populations become so distinct that if you cross-bred between them, then no viable offspring could be produced. This would mean that they had become two separate species.
