You should check your head gasket. You can go to Autozone and rent for free head gasket checker. You have to buy the fluid. You basically are checking for exhaust in the coolant. It should tell you if you have a head gasket failure. Also check your oil to see if it has milky quality. If so, then head gasket. Also, you did not mention changing the thermostat.
