Indoor case sheet or patient case sheet is the informational sheet of a the complete surgery done in a hospital and its maintained by the hospital, this carries the entire treatement information right from the joining to the discharge of the patient.
It should only need to be done once ! The patients belongings should have been catalogued on arrival at the hospital. That list should be compared with the list drawn up before the patient leaves to go to the new hospital. Once the patient arrives at the new hospital - they are no longer the responsibility of the current assistant. The staff at the new hospital should catalogue the patients belongings, and compare them to the current list.

All new patients should receive Privacy and Disclosure notices on the first visit to a doctor's office, hospital, out-patient service, etc. These are usually given and patient signature required before seeing a physician, dentist, or other health professional.
