You can ask yourself if you are preoccupied with anything or doing something to the point that it interferes with your life. If you are really scared of something that you cannot leave your house, you would know it. If you are to a point where you don't know if you can cope anymore, you would know it. You can ask yourself if you want to harm yourself or another, and if so, you can go get help. Or you can ask yourself about your grip on reality, or if you find yourself with baseless fears or making baseless accusations. You should also ask yourself how well you are sleeping and what is your relationship with food. If you cannot sleep, sleep all the time, refuse to eat, vomit what you eat, or refuse to stop eating, then you should seek help.

If you don't have the ability to examine your life and behavior, then others will tell you how odd your life is. They may be concerned about your safety and their own. They may notice that you are having paranoid delusions like someone is out to get you, and such a preoccupation in your mind interferes with your life. Patients like this are affected in the part of the brain that would tell them they are ill, so they would need outside intervention.
