No, not necessarily. Imagine the US president, and the Queen of the UK. Both are democratic nations - the Queen is the UK's figurehead, the Prime Minister is a closer equivalent to the president. But then also consider the so-called 'President for Life' of a dictatorship, or a tyrannical despotic monarch. The average citizen will have a much better quality of life in the first two cases, than the second. So the title itself is not as important as the nature of the leadership and government.
