I have the same problem with my 99 Malibu heater... I'm not sure what the cause is, but I have found that if you run the heater in the recirc mode for a little while, it will start to heat up after 5 to 10 minutes and you can switch it back to the fresh air mode. My Malibu also used to run hot after it was about 6 years old. I found the radiator and block had a fair amount of sludge from that "lifetime" coolant GM uses. I flushed the engine and radiator till it was clean and replaced it with regular anti-freeze. Hasn't been a problem since.
