Chief of Staff is the title of the highest ranking officer of the US Army. He holds the rank of General, which is an 0-10 / 4 star rank. The title of Commander-In-Chief is given to the President of the US and is the commanding authority for all military forces, but the holder of this rank, while often having served in the military, is not a military officer by virtue of title.

The US Military never did away with the rank of Five Star General. It's just that no one has been promoted to the rank since perhaps Eisenhower.
None has ever been awarded that rank by the US Government.

typically the highest rank is a General (aka 4 star General), but in some cases one officer is promoted to General of the Army (5 star General) and in extremely rare cases (only twice in US history) a General can attain the rank of General of the Armies, which is senior to all officer ranks past and present.
Five Star General - GOAF (General of the Air Force) this is reserved for wartime only. Four Star General - during peacetime this is the highest ranking officer in the Air Force. But if you're talking about highest officer rank that is NOT a General rank. It would be Colonel.

They are George Marshall (December 16, 1944), Douglas MacArthur (December 18, 1944), Dwight D. Eisenhower (December 20, 1944), Henry H. Arnold (December 21, 1944), and Omar Bradley (September 22, 1950). General of the Armies is a special honorary 5-star rank which has been confirred only two times. The formal rank is General of the Armies of the United States. Two US Soldiers have held this rank: George Washington and John J. Purshing. Some historians have referred to this at a 6-star general to distinguish the rank from a 5-start General of the Army (not Armies).
