Apollo 17 was the sixth and final lunar landing mission of the Apollo program. Launched on the 7th December 1972, and returned to Earth on the 19th December. It remains the most recent manned moon landing and the most recent manned flight beyond low Earth orbit.
NASA's Phoenix lander is currently on Mars. The most recent mission prior to that was a NASA orbiter in 2005. The most recent landing prior to the current was in 2003. A link below has a list of all Mars missions.
There have been no astronauts on Mars. Probes have been sent there, but it will be a long time before a manned mission will be sent there.
No human has yet landed on Mars, though at least half a dozen unmanned probes have. Plans are in motion to put people on Mars sometime during the next decade.
