Usually never but there are rare cases that i have heard of but that is when the state is no longer part of the nation,or there are more people there than the nation can over power. An example of this is when there are gangs or mafias like the KKK.

In the Constitution the states were intended to have more power, and now National Government has more power but not as much power as a State government has on a State. In other words National government has more power to the country but the state government has more power to the state.
State government deals with its constituents' issues within the state. There are also more local governments, as well as a national government.

Neither, since confederation and government are two different concepts. One describes a regime type and the other is the lowest level of the state.
Basically that is how government operates today, although local and state governments have some independence, there is no true independence. This is good and bad, from a historical standpoint of court cases local laws are often far more oppressive than national laws but governments on a local level are much easier to manage than national ones.
