Sounds fishy to me. If you are regularly employed and receive a wage and do not fall under the classification of either an "independent contractor" or a "casual laborer," you should be covered under the Workman"s Compensation Law of your state.
It's a possibility that your employer may not be complying with the law - and the matter of your taxes being reported and withheld may also have to be addressed as well. I would recommend retaining counsel to protect yourself - if your employer is violating the law(s) he will resist your allegations.
You would be responsible for your own medical bills. You are probably being treated as a self employed taxpayer (contract worker). You will have to deal with the payer of your services about this matter.

If you can prove that you ARE are employed - and receive a wage - you should be under the auspices and protection of the Workman's Compensation Act for any on-the-job injury. The matter of no W-2 is a tax matter and will have to be addressed seperately.
Nope no degree needed, but it doesn't hurt to take a class and get certified.
