The manufacturer will refer you to someone locally who can remove it for you. The Doctor Who inserted it should be able to refer you to someone as well. Your local family planning office will have experience in removals as well.
That is not a problem. If problem persists, you can consult the gynaecologist.

I think this depends more on the person than the implants. Some people have more of a problem with tissue "absorption" than others. I imagine a softer or smaller implant would result in less of this. This phenomen can be a problem with any implant or piercing, including even lens implants for the eye.
