I'm updating this answer because I have had to do this several times (unfortunately). I have never gone physically to revenue Canada.
# Phone 1-800-959-8281 (or if this answer is now old, go to the revenue Canada website and look for the general inquiries phone number) # press the * key right away to speak to someone. # tell them you want a t4 for whatever year. # They'll ask you your SIN number and who you are, where you live etc. # Then they ask which employer you need the t4 from and voila! # in 8 to 10 days, you receive your t4 in the mail.
Today (Friday morning) it took me less than 5 minutes and the phone representative was super polite. (NOT like revenue Quebec!!!)
