There are three different ways to receive a copy of your official state of Florida birth certificate if you are a Florida native and were born after 1917. You can order by mail, by fax, or at the Bureau of Vital Statistics offices. Before 1917, only photocopies are available, not a computer record certification.

Birth records in Florida can only be accessed by the registrant if 18 years or older, to the registrant's parents, guardian or legal representative, or by court order. You can download an application from the Florida Department of Health, Bureau of Vital Statistics website and include a copy of current photo ID of the applicant. If you are requesting your child's certificate, include a copy of your own ID since you are the applicant. Applications are accepted in person or by mail at the State Bureau of Vital Statistics, or by phone at (877) 550-7330.

A computer generated certification with the embossed Seal of Florida is available from 1917 to present and is accepted by all state and federal agencies as legal proof of birth and citizenship. Only photocopies are available for births prior to 1917.
