It's easy to create a formal place setting. The items used in a formal place setting depend on the courses served. Only utensils for the dishes being served are placed on the table. From left to right: A fish, meat, and salad fork are placed in that order to the left of the service place which is in the center of the place setting. The salad, meat, and fish knives are placed on the right side of the plate and correspond to the forks. The soup spoon is to the right of the knives. An oyster fork is added by the soup spoon if oysters are served. A napkin is place in the center of the service plate and a place card goes behind the service plate. Glasses include a water goblet, red and white wine, and a sherry glass. Champagne may also be included. Glasses are placed to the top right of the service plate. Coffee comes out with dessert.
The place plate is the basic plate of an individual table setting. It is used as a marker or guide for the silverware and other components of the place setting and functions as a service plate when the food is serviced.

There are different table settings for breakfast, lunch and for dinner. The formal table setting includes the wine and water stemware, linens, butter and bread plate, salad plate and the dinner plate.
I paid $49.95 for four place settings. Each place setting included a dinner plate, salad plate, cereal bowl and a mug.
You place the water glass on the right of the plate in the "corner" of the place setting.
Typically, the folded napkin is placed on the left side of the place setting, either on the plate or next to the fork.
the i have no idea side the bread plate is placed to the right of the dinner plate, unless it is a very formal setting where the bread plate will be above the forks to the left of the dinner plate. The salad plate is always to the left of the dinner plate when not stacked on top of the dinner plate as in a formal setting.
In a formal dinner setting utensils are either on the left or right of the plate. The knives and spoons are set to the right of the plate and the forks are set to the left of the plate. If there is more than one fork like a salad fork, the salad fork will be placed further left to be used first. The same applies to a soup spoon which will be placed further right of the plate to be used first.
To do the Russian style table settings, you would place a doily or some other cloth over the table cloth at each place setting. Next place a charger plate, the dinner plate goes over the charger plate. Silverware is placed on the table in the order that it will be used. A bread and butter plate with a butter knife is placed on the table over the forks.
The dinner knife, or 'service knife,' in a semi-formal, or formal setting would be for the knife to be at the right of the dinner plate, with the blade facing the plate. If there is a butter plate and butter knife or 'spreader,' that knife should be on the butter plate to the left of the dinner plate, just above the cutlery on the left. The butter knife should be place with the handle facing to the right and the blade facing downwards. If there is an additional knife, such as a fish knife, etc., that knife should be to the right of the dinner knife, with the blade facing towards the dinner knife.
