If you used disposal tap for dishwasher drain there is a knock-out inside the tap that must be removed.Take a large screw driver and hammer and remove knock-out.

You can, but depending on the model you may have problems with water siphoning out of the dishwasher during operation. Putting in a high loop may prevent this, and running the drain to a countertop air gap device before passing through the floor should certainly prevent it.
