Providing that you have done all you need to do on the Dragon Slayer quest, youwill go onyour boatand it sails you to Crandor, you have to go up to the top of the volcano (There is Hill Giants and Lesser's) you have to follow a path in the volcano and elvarg will appear. Jump over the ledge and battle her.

Yes, you can wear rune and fight green dragons. I'm guessing this question is pertaining to Elvarg more than just regular green dragons. If that's the case, you can still fight Elvarg while wearing rune, you just can't wear the rune plate as you have to defeat Elvarg in order to wear it after returning to the Hero's Guild for your reward.
On runescape How do you open the wall in karamja volcano?
You need to have completed the quest: Dragon Slayer, from this quest you will gain access to the wall, the wall leads to the volcano where you fought Elvarg, from there you can go up a rope to the island of Crandor.
