It only appears on your world map after you find Brother Genitivi's research notes. His research can be found in a house in Denerim, across from 'The Gnarled Noble'. You'll encounter a man there who tells you he's watching the house for brother Genitivi. You can ask him a number of questions regarding the Sacred Urn but if you try to enter the bedroom he'll stop you asking you not to go in there because he feels you'll mess up all the books and papers. Tell him you're going to look anyways and he'll attack (there's no way to avoid attacking him). Then enter the room and search everywhere you can. When you leave Denerim, Haven should appear on your world map.
