Both terms are correct. It depends on the circumstances you are using them.

When you are talking about something that happened in the past you should use "did you want?". For example: Did you want to go out with your friends instead?

When you are presently talking to someone you should use "do you want?". For example: Do you want to go out to dinner or stay and watch a movie?
