There have been a series of robotic probes to every planet in the solar system, and the two Voyager probes are approaching (or perhaps have just passed, we're not yet sure) the "edge" of our solar system. But no human has ever been further away than the Moon.
As far as has been publicly announced, man-made spacecraft have landedon the earth, on the moon, and on Mars.

It's not clear what you mean by "visited", here. All man-made spacecraft ORIGINATED from Earth. Some of them later RETURNED to Earth as well, and quite a few of them were designed to STUDY Earth (for example, weather satellites).
