Generally speaking IELTS is an exam used mainly for academic entry which validity expires in two years time, whereas CAE is a certificate for a lifetime that states that you can practically use English in C1 level(eventhough if you want it for entry the period stays the same). This means that for CAE you must cover all the material for C1, which is far more difficult than your preparation for IELTS. You don't have to study grammar, vocabulary,idioms, develop writing skills for different writings(report,proposal,article etc.), you just need to understand English in its Intermediate level and talk relatively good. DO IELTS!

No, it is not a must. It is the most popular exam for university admission of foreign students however there are other exams that are accepted by universities - IELTS, CAE, CPE! You have to check your prefered university requirements.
