Gerald Ford had two attempts made on his life while he was President. The first was on September 5,1975 by Lynette Fromme. She had a gun in her hand but was restrained by guards before she fired a shot.

The second attempt was on September 22, 1975 by Sarah Jane Moore. She got off a shot from 40 feet away but missed when a by-stander grabbed her gun.

The president who survived two assassination attempts was Gerald Ford.

The first assassination attempt was by Lynette "Squeaky" Fromme, on 5 Sept 1975 in Sacramento California. Fromme was a follower of Charles Mansion. A Secret Service member stopped her from firing her pistol. She was sentenced to life in prison, but was recently paroled on 14 Aug 2009.

The second assassination attempt came about three weeks later, in San Francisco California--when Sara Jane Moore pointed a pistol at the President. A bystander saw her take aim, grabbed her and deflected her shot. She also was convicted and sentenced to life in prison, was paroled 31 Dec 2007.

Three possible answers:

1)) Richard Nixon: survived two attempts, one in 1972 when a Milwaukee native tried to shoot Nixon on a trip to Ottawa, and one in 1974 when a 9-11-style one-man hijacking failed to materialize.

2) Gerry Ford: Two attempts about two weeks from each other in September 1975, both in California, the first by a member of the Manson family.

3) George W. Bush: in February 2001, a man fired shots at the White House from behind the fence in front, but it was a pretty half-assed assassination attempt as far as things go. The second was when a live grenade was tossed his way in 2005 during a visit to Georgia (the country, not the state).
