Because an ultrasound at 7 weeks and one at 18 weeks use different methods for estimating gestational age, they have different levels of accuracy. In general, a skilled sonographer's 7 week ultrasound is more accurate for dating than a skilled sonographer's 18 week ultrasound. A sonogram is 100% accurate, insomuch as it's basically a picture. Interpretations of that picture may vary, and may or may not be accurate, but the sonogram itself is a sounding of whatever is being examined, incapable of being "inaccurate."

If the ultrasound is done in the first weeks of pregnancy it is possible to date the embryo within a few days, an ultra sound done later is not as accurate for dating purposes.
Did you ask your doctor or sonogram technician this question? I do not believe that it would indicate a birth defect. My son had short legs (and still does). I wouldn't worry, but I would ask the sonogram technician to explain. Good luck :)

There is no point in doing a pelvic exam at 5 weeks. A sonogram will show if you are pregnant.
An internal sonogram can show pregnancy as soon as 4 weeks. There will likely show an amniotic sac which is the first to form during pregnancy. Within a week after that you can see the fetus start to form. The heartbeat will likely show around 8 weeks.
