Depends on patient condition. If there is an immediate threat to life or limb, then the lights and siren should be used. Otherwise, they shouldn't. They come with an inherent risk of distracting other drivers and possibly causing accidents, and in today's "I'm the most important person in the world" society a lot of people won't even pull over anyway. Oh, and everywhere has the same laws - if you're running either lights or siren, you run both. Never run a silent lights, because it's illegal.

If the victim is found unconscious, we cannot rule out a head/spinal injury. As such, we use the clamshell to immobilize the patient, then lift them onto a 35A stretcher for exchange with the ambulance.
Sime to not alter the position of the victim so as to do no more harm and to use a backboard to immobilize their movement while transported in a horizontal position such as in an ambulance
