Ronnie O'Sullivan holds the record. 5 minutes 20 seconds, against Mick Price. I think it was at the Crucible, Sheffield, in the world championships. It is unlikely that this will ever be beaten.

The first maximum break was made by E.J. Murt O'Donoghue at Griffith, New South Wales, Australia on 26th September 1934. Joe Davis compiled the first officially recognised 147 against Willie Smith on 22 January 1955 at Leicester Square Hall, London. The first maximum compiled in professional competition was made by John Spencer in 1979, but it was not officially ratified due to oversized pockets.

The first televised 147 was by Steve Davis in the 1982 Lada Classic.

Cliff Thorburn made the first World Snooker Championships 147 break (Maximum) on 23rd April 1983 against Terry Griffiths. The Canadian's maximum took over 30 minutes to complete, maybe the slowest in history, and was famous for the fluked red that started the break.
