Knights on horseback were like a tank to the middle ages. They had horses that were trained to carry a man in armour and to be controlled with the knees. They would form a line knee to knee and with the full armour they were very powerful.

To be put on a horse he had to be lifted on to the horse. The helmet also didn't allow for the full range of view so in a fight this would limit him. Finally, it was expensive to purchase.
That depends on the time period. Officially, the medieval period lasted from the 11th century through to the 16th century.
