Remembering your manners is all about respecting those around you. Constantly remember that. If you remember that, you will remember the common knowledge of manners.
A gentleman is defined by manners and class. Often a person with money and manners is considered an gentleman, however saying this there were those who were very poor but still had their manners who were considered gentlemen because they were so nice;
Manners are very important to those around you. It shows that you have class and respect for others. Many jobs love when employees have good manners. This includes respectful communication (yes sir, no sir), good tables manners, and wearing appropriate attire.
Some days it may seem so, so it is left to those of us who were taught manners as a child to promote them and lead by example. Good manners make any social interaction easier.

When I was growing up, it was "great uncle". We have a lot of fun with this, as in "I'm going to ask the Great Uncle Mark if I can have some ice cream." Anyway, to answer your question, we always use great uncle or great aunt when referring to those relatives.
